📋 Description

• Conducts initial in-home clinical assessments and coordinates care activations.
• Coordinates activation and ensures required forms are completed.
• Develops, implements, and manages patient care plans.
• Provides patient and caregiver education and supports quality assurance.
• Collaborates with patients, caregivers, and interdisciplinary teams.
• Ensures safe, person-centered care and regulatory compliance.

🎯 Requirements

• Valid and active RN license in the State of Colorado.
• Ability to articulate cases with effective summaries.
• Ability and willingness to travel; valid driver’s license.
• Proficient in Case Management Systems and MS Office.
• Strong customer service with empathy and active listening.
