Provide hands-on personal care and ADLs to participants at home
Assist with instrumental ADLs such as laundry, light housekeeping, shopping, and meal prep
Measure and document vital signs as instructed
Act as medical escort to appointments, aiding safe transfers
Support Rehabilitation team by carrying out home exercise programs per care plan
Provide translation services as needed

🎯 Requirements

High school diploma or equivalency
Current BLS/First Aid Certification required
CNA or HHA certification preferred
Bilingual preferred
Must be registered with Home Care Registry prior to start date
CPR and First Aid certification within 180 days of hire and maintain
