📋 Description

• Coordinate care across settings (inpatient/outpatient/ER/home care) with confidentiality.
• Engage members and providers to create care plans and navigate the healthcare system.
• Assess and support SDOH needs; educate on community resources.
• Participate in crisis intervention for members.
• Advocate for members and address barriers to care or adherence.
• Educate members on health conditions, meds, and procedures via in-person, phone, or secure messaging.

🎯 Requirements

• 2+ years medical social work in a healthcare facility setting.
• 1+ years experience in Care Coordination and Navigation.
• Masters in Social Work (MSW) from a CSWE-accredited program.
