Case Manager

📋 Description

• Assist individuals in identifying and acquiring natural and paid supports to live as independently
• Function as an educator, resource, and service coordinator in a person-centered approach
• Coordinate services for vulnerable individuals and help navigate the service delivery system
• Provide ongoing assessment, planning, referral, service coordination, monitoring, consultation, and
• Directly manage a caseload and coordinate supports to positively influence clients' lives
• Maintain professional contact with organizations, families, and community members

🎯 Requirements

• Bachelor's degree in social work, psychology, sociology, or closely related field, or licensed
• 1 year of related work experience
• Valid driver's license with acceptable driving record
• Proof of automobile insurance
• Knowledge of the local community
• Good oral and written communication skills
