Case Manager II

📋 Description

• Coordinate and manage a caseload of at least 15 clients with trauma-informed care.
• Facilitate assessments, service plans, and progress notes in ONE System and Salesforce daily.
• Lead weekly workshops/psychoeducation groups for housing access and recovery.
• Identify participant strengths and barriers; connect to internal/external resources.
• Document outcomes and determine case closure within agreed timelines.
• Collaborate with supervisor; participate in trainings and community events.

🎯 Requirements

• Minimum 3 years' experience in recovery-oriented field or related bachelor's.
• 2+ years’ experience with marginalized populations.
• Experience in case management and group facilitation; interview and assess clients.
• Strong verbal and written communication; ability to draft service plans.
• Proficiency with Microsoft Office; ONE_System and Salesforce preferred.
• Certification: Certified Peer Specialist or SUD, or willingness to secure within 90 days.
