📋 Description

Conduct clinic nursing assessments to determine participant needs and goals
Participate in IDT meetings to develop and adjust care plans
Deliver nursing interventions and administer medications per care plans
Coordinate participant care with IDT and families
Educate participants and families on disease processes and self-care

🎯 Requirements

Graduate of accredited nursing program; BSN preferred
Unencumbered California RN license
Current BLS / First Aid certification required
Two years nursing experience with frail/elderly population preferred
CPR and First Aid certifications within 180 days of hire
