Case Manager, RN

📋 Description

• Conduct nursing assessments to guide care plans with IDT
• Participate in IDT meetings; develop participant care plans
• Deliver nursing interventions; administer meds; ensure safety
• Coordinate care with PACE physician and IDT; update families
• Educate participants and families on disease, self-care, prevention
• Support transitions of care and ongoing patient advocacy

🎯 Requirements

• Graduate of accredited nursing school; BSN preferred
• Unencumbered California RN license
• Current BLS/First Aid certification
• Two years nursing experience with frail/elderly population preferred
• CPR and First Aid within 180 days of hire (cert preferred)
