Case Manager, RN

📋 Description

• Guide participant care as the voice on the IDT.
• Conduct PACE assessments and develop care plans.
• Deliver nursing interventions and manage medications per plans.
• Collaborate with IDT and families to revise care plans.
• Educate participants and caregivers on disease, self-care, and prevention.

🎯 Requirements

• Graduate of accredited nursing school; BSN preferred.
• Unencumbered California RN license.
• Current Basic Life Support (BLS)/First Aide Certification required.
• Two years nursing experience with frail/elderly population preferred.
• Must obtain CPR and First Aid within 180 days of hire; maintain certification.
