  • Conduct in-home clinical assessments and help patients and caregivers begin services.
  • Develop, implement, and update individualized care plans based on patient needs.

🎯 Requirements

  • Graduate of an accredited nursing program.
  • Active and unrestricted RN license in Colorado.
  • Strong clinical assessment, communication, and documentation skills.
  • Compassionate, patient-centered approach to care.
  • Ability to travel by car within an assigned geographic area.
  • Comfort using common clinical, case management, and productivity software.
