Case Manager, RN Contract Opportunity: $200 per visit (Denver, CO / Field-based)

📋 Description

• Conduct in-home clinical assessments and start services.
• Develop, implement, and update plans of care based on patient needs.
• Build relationships with patients and caregivers through visits.
• Coordinate care with physicians, teams, and resources.
• Educate patients and caregivers on care plans and safety.
• Review caregiver documentation and respond to clinical concerns.

🎯 Requirements

• Graduate of an accredited nursing program.
• Active and unrestricted RN license in Colorado.
• Strong clinical assessment, communication, and documentation skills.
• Compassionate, patient-centered approach to care.
• Effective communication with patients, families, and care team.
• Ability to travel within assigned geographic area.
• Valid driver's license and auto insurance.
