Case Manager, RN (Portland, OR / Field-Based)

📋 Description

• Conduct initial in-home assessments for new patients.
• Orient patients, caregivers, and PCAs; provide ongoing care.
• Coordinate care with interdisciplinary teams; schedule home visits.
• Develop individualized plans of care; monitor outcomes.
• Ensure regulatory compliance and accurate documentation.
• Communicate clinical concerns and barriers to the care team.

🎯 Requirements

• Active RN license from Oregon Board of Nursing.
• Diploma from accredited nursing program.
• Care coordination and client documentation experience.
• Proficient with Case Management Systems and MS Office.
• Bilingual English/Spanish preferred.
• Strong communication and customer-service skills.
• Ability to travel within assigned geographic area.
