CCM Case Management RN

📋 Description

• Coordinate care across ER, inpatient, post-acute, outpatient, and home care
• Educate members on health outcomes and guide them through the care system
• Biopsychosocial needs assessment and outcome-based care planning
• Develop outcome-based care plans with targeted dates for completion
• Conduct in-home and facility visits to engage and educate members
• Discharge planning and advocacy for appropriate level of care

🎯 Requirements

• 2+ years of clinical experience
• Active, unrestricted RN license in your state of hire; ability to obtain additional licensure as needed
