Central Pre-Enrollment Provider Nurse Practitioner

📋 Description

• Conduct chart reviews for PACE applicants evaluating records, functional status, and docs.
• Present cases to Site Medical Directors to support eligibility decisions.
• Collaborate with Enrollment RNs and Site Medical Directors to resolve documentation gaps; escalate as needed.
• Provide clinical input for Health & Safety determinations; conduct virtual assessments when gaps exist.
• Review ILOC summaries to connect medical conditions with PACE services.

🎯 Requirements

• Current, unrestricted NP or PA license in California
• Current CA DEA certification and prescriptive authority
• Minimum 3 years post-licensure NP clinical experience in geriatrics/long-term care/PACE-adjacent
• Demonstrated chart review proficiency for PACE eligibility and H&S denial
• Proficiency with geriatric tools: BIMS, PHQ-9, MoCA, MMSE, ADL/IADL
• Proficiency with Epic EHR and Microsoft Outlook; manage high-volume communications
• Understanding CMS and DHCS regulations related to PACE eligibility and NFLOC
• Ability to collaborate with Habitat Health PACE Site Medical Directors across multiple CA sites
