Centralized Pharmacy Liaison

📋 Description

• Provide full medication management and patient care.
• Collaborate with clinicians to serve patients in person and by phone.
• Educate patients on hospital pharmacy services and care.
• Support outpatient retail and specialty prescriptions.
• Ensure patients stay on track with regimens; manage refill reminders.
• Identify storage and handling needs for hazardous/refrigerated meds.

🎯 Requirements

• State-registered pharmacy technician; CPhT preferred.
• 2 years minimum; 3-5 years preferred.
• High School Diploma or GED required.
• Proficient with Microsoft Office (Excel, Word, PowerPoint) and data entry.
• Strong interpersonal and communication skills; ability to work independently; good judgement.
• Knowledge of transplant/hepatitis C/infectious disease/oncology a plus.
