Clinical Manager, Home Health

📋 Description

• Lead clinical staff to deliver outstanding patient care
• Supervise RNs, PTs, OTs, SLPs and other staff
• Conduct assessments and supervisory visits in the service area
• Develop care plans and oversee services
• Facilitate new hire orientation
• Ensure accurate clinical documentation

🎯 Requirements

• Minimum 2 years RN experience
• Home Health and OASIS experience (1 year min)
• Acute care, medical-surgical or sub-acute experience a plus
• Strong clinical, communication and organizational skills
• Ability to build relationships with clients, families and teams
