Conduct comprehensive client assessments in home care and develop care plans.
Lead and oversee care plans, ensuring safe, high‑quality care and regulatory compliance.
Provide clinical oversight for assigned caseload and ensure timely documentation.
Collaborate with physicians and providers to coordinate care and document orders.
Perform clinical visits and supervise reassessments as required.

🎯 Requirements

Education: RN licensed in good standing in Arizona.
Experience: Minimum of two (2) years clinical experience; home care/med-surg/LTC preferred.
Open to new grads/newly licensed RNs; pediatric experience for pediatric clients.
