Clinical Pharmacist

📋 Description

• Functions as the medication expert on the interdisciplinary care team.
• Reconciles and optimizes COPD-related med regimens and adherence.
• Delivers evidence-based, patient-centered medications recommendations with providers.
• Delivers COPD-focused disease state management, inhaler technique education, and adherence support.
• Identifies high-risk COPD patients via population health platforms and coordinates follow-up.
• Documents encounters in the EHR and supports quality improvement initiatives.

🎯 Requirements

• PharmD degree with two years of professional experience OR BS in Pharmacy with three years.
• BPS certification or ASHP-accredited residency in area.
• Active Rhode Island pharmacist license.
• Ambulatory care or population health experience preferred.
• Experience with EHR, population health platforms, and reporting tools.
• Demonstrated ability to work in an interdisciplinary care team.
