Clinical Pharmacist

📋 Description

• Review clinician notes for prior authorization and care plan recommendations.
• Develop and refine disease- and drug-specific clinical protocols.
• Monitor patient adherence (PDC, assessments, mobile data) across sites.
• Evaluate EMR for drug interactions, labs, allergies, and orders.
• Recommend guideline-based alternative regimens to optimize care.
• Medication therapy management (MTM): review regimens and interventions.

🎯 Requirements

• PharmD or equivalent; licensed Pharmacist in Florida.
• Florida license required; 1+ year experience preferred.
• Strong knowledge of specialty pharmacology and regulations.
• Proficient with Microsoft Office; EPIC/QS1/WILLOW a plus.
• Excellent communication and patient coaching skills.
• Ability to maintain HIPAA compliance and confidentiality.
