Clinical Pharmacist (Hematology/Oncology)

📋 Description

• Conduct comprehensive assessments for disease management under collaborative practice agreements or
• Work on-site in close coordination with interdisciplinary teams, including both in-person and
• Assist in the development, maintenance, and appropriate use of treatment plans and supportive care
• Monitor and evaluate medication regimens to ensure clinical criteria are met, including therapeutic
• Ensure all clinical criteria are in compliance with FDA standards, national clinical guidelines
• Identify, resolve, and prevent medication-related problems to ensure safe and effective

🎯 Requirements

• Doctor of Pharmacy (PharmD) and active Louisiana pharmacist license required
• Oncology Pharmacy Specialty Certification (BCOP) required within 1 year of hire
• PGY1 + PGY2 residency in area of specialty/related field OR PGY1 + 1 year practice experience in
• Strong clinical judgment, communication, and process management
• In-depth knowledge of pharmacology, FDA regulations, and national treatment guidelines
• Proven ability to ensure legal compliance and deliver high-quality, detail-oriented care
