📋 Description

• Provide telephonic interventions to improve medication outcomes.
• Counsel patients on adherence, address barriers, and document commitments.
• Perform focused chart reviews to inform interventions.
• Escalate complex cases to prescribers or care teams.
• Collaborate with physicians and staff to optimize medication workflows.
• Aim for performance goals (STARs, HEDIS) in quality measures.

🎯 Requirements

• PharmD degree.
• Active, unrestricted pharmacist license; multi-state licensure in at least 2 states.
• Willingness to obtain additional licenses within 6 months.
• Minimum 3 years in value-based care, ambulatory, or community pharmacy.
• Strong verbal and written communication; translate clinical data clearly.
• Proficiency in telephony platforms and EHR systems.
