📋 Description

• Provide comprehensive medication management, including reconciliation, dose optimization, adherence
• Partner with physicians, specialists, and interdisciplinary teams to order individualized care
• Monitor therapy effectiveness and safety, identify optimization opportunities and intervene as
• Support quality-improvement and REMS initiatives.
• Provide recommendations to operations by supporting teammates as needed with complex prior

🎯 Requirements

• Hold a Doctor of Pharmacy (PharmD)
• Must be a licensed pharmacist in Washington
• Completion of a PGY2 Ambulatory Care Residency, OR Completion of a PGY1 Residency and at least 1
• Strong communication, organization, and tech skills
• Empathy, integrity, and a team-first mindset
