Clinical Pharmacist (Temp-to-Hire)

📋 Description

• Engage patients by phone to deliver Arine's clinical services.
• Conduct MTM services: CMRs, TMRs, adherence counseling, and reconciliation.
• Document patient data in Arine's software; prepare reports on care and team progress.
• Follow up with clinics, providers, pharmacies to resolve issues and transitions of care.
• Develop intervention strategies to optimize medication use and meet quality measures.
• Work independently with accountability and ongoing coaching.

🎯 Requirements

• PharmD from an accredited college/university.
• Licensed Pharmacist in good standing; multi-state licenses a plus.
• Knowledge of CMS quality measures, Star Ratings, and Medicare Part C/D.
• Knowledge of chronic disease management and polypharmacy.
• Ability to review prescriptions considering age and comorbidity.
• MTM platforms experience; remote/phone-based care.
• Strong computer skills: Google Workspace, Mac, Slack, Five9.
• Must reside in the United States and be eligible to work; willingness to obtain additional licenses.
