Clinical Pharmacist (Temp-to-Hire)

📋 Description

• Phone-based MTM services delivering patient education
• Conduct Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs)
• Document patient data in Arine's software; prepare reports
• Coordinate with clinics, providers, and pharmacies to resolve issues
• Support transitions of care and care plan education

🎯 Requirements

• Doctor of Pharmacy (PharmD) from an accredited college/university
• Hawaii Licensed Pharmacist in good standing
• Knowledge of CMS quality measures, Star Ratings, and Medicare Part C and Part D
• MTM experience including CMRS and TMRs
• Excellent written and oral communication; patient-focused
• Strong computer skills; Google Workspace, Slack, Five9
• Ability to work remotely; 40 hours/week; Pacific Time friendly
• Willingness to obtain licensure in other states as needed
