Clinical Pharmacy Specialist

📋 Description

• Provide comprehensive medication management including reconciliation and dose optimization.
• Partner with physicians and care teams to order CPA-based care plans.
• Monitor therapy effectiveness and safety; intervene as needed.
• Support quality improvement and REMS initiatives.
• Assist operations with prior authorizations, workflows, and patient education.

🎯 Requirements

• Hold a Doctor of Pharmacy (PharmD).
• Must be licensed as a pharmacist in Oregon.
• Completion of PGY2 Ambulatory Care Residency.
• Completion of PGY1 Residency and at least 1 year ambulatory practice under a CPA.
• At least 3 years of ambulatory practice with disease state management.
• Strong communication, organization, and tech skills.
• Empathy, integrity, and a team-first mindset.
