📋 Description

• Review and ensure accuracy of OASIS documentation for SOC, Recertification, and Discharge
• Ensure timeliness and CMS/compliance for OASIS items.
• Provide education and guidance to clinicians and clinical managers on documentation practices.
• Collaborate with Home Health Senior Care Clinical Quality Team to maintain standards.
• Review 485 plans of care for SOC and Recertification assessments.
• Identify documentation trends and opportunities for process improvement.

🎯 Requirements

• Active RN or Physical Therapist license with compact licensure or ability to obtain in the hiring
• 3–5 years of home health experience.
• Experience as Clinical Manager or OASIS Reviewer.
• Strong knowledge of Medicare home health regulations, OASIS-E guidance, and PDGM requirements.
• Experience with 485 plan of care review, medication reconciliation, and documentation compliance.
• Proven ability to educate clinicians and clinical managers on documentation best practices.
