📋 Description

• Provide home health care on a per visit basis in Nassau County/North Jacksonville, FL
• Assist with activities of daily living as needed
• Light housekeeping and home management tasks
• Monitor and report changes in patient condition
• Ensure patient safety and comfort in home settings
• Transport coordination and scheduling with BAYADA team

🎯 Requirements

• Florida CNA license or Home Health Aide Certificate
• At least one year of work experience
• Listed on Florida Nurse Aide Registry, active/in good standing
• English communication proficiency
• Pass BAYADA Competency Evaluation
• Criminal background check and state requirements met
