Community Health Outreach Coordinator (Part Time, Spanish Speaking)

📋 Description

• Identify CBOs, FBOs, FQHCs, and providers serving SCFHP members in Santa Clara County.
• Identify and evaluate local events, health fairs, and clinic opportunities for Foodsmart.
• Attend enrollment events to raise awareness of Foodsmart's foodcare program and SCFHP benefits.
• Cultivate relationships with organizations to build awareness of Foodsmart.
• Define scope of partnerships, including onsite events and referral pathways.
• Work with marketing to create multilingual event materials (flyers, recipe cards, confirmations).

🎯 Requirements

• Based in Santa Clara County and able to travel throughout the county and Bay Area.
• Bilingual in English and Spanish (required).
• Proactive and community-rooted; advocate for the populations served.
• Comfortable handling confidential and sensitive member information.
• Proficient in Microsoft Office and CRM tools (ReferralMD, Salesforce, or similar).
• Strong organizational skills and ability to track event enrollment, referrals, and outcomes.
