🎯 Requirements

Based in Santa Clara County and able to travel across the Bay Area as needed.
Bilingual in English and Spanish (required).
Proactive, community-rooted advocate for the populations served.
Proficient in Microsoft Office; comfortable with CRM tools (ReferralMD, Salesforce).
Able to travel to events, health centers, and FQHC sites in Santa Clara County.
Strong organizational skills with tracking of enrollment, referrals, and partnerships.
