Community Health Worker I

📋 Description

• Provide direct participant support to address SDoH needs through home visits, outreach, and
• Connect participants to established community resources (e.g., housing supports, food insecurity
• Assist participants with completing and submitting applications for identified community resources
• Support Social Workers in discharge planning by supplying applicable community resource information
• Support participants with complex needs through ongoing engagement, resource connection support
• Collaborate with Social Workers, ECAs, PELs, and the broader IDT to reinforce participant-centered

🎯 Requirements

• High school diploma or equivalent required
• Associate's degree in social work, human services, social science, or related field preferred
• Reliable means of transportation
• Strong knowledge of community-based services and resource navigation
• Experience working with older adults or vulnerable populations is a plus
• Ability to independently manage tasks with minimal supervision
