Community Health Worker, Manager - San Bernardino/Orange County

📋 Description

• Provide direct supervision of CHWs in Orange and San Bernardino counties
• Lead weekly multidisciplinary team huddles to coordinate care
• Coaching and mentorship to CHWs; ensure competency validation
• Manage staffing schedules for patient coverage and continuity
• Review data dashboards to monitor CHW performance
• Represent Waymark in the community and build partnerships

🎯 Requirements

• Experience supervising teams of CHWs or related health workers
• Experience delivering health services as a CHW or similar role
• Strong understanding of Medicaid and access barriers
• Good judgment and empathetic team leadership
• Comfort using technology for virtual communication, scheduling, documentation
• Ability to travel locally within Orange and San Bernardino counties
