Community Health Worker, Manager - Santa Rosa/Sonoma County, California

📋 Description

• Lead and supervise CHWs in Healdsburg and Santa Rosa areas.
• Coordinate weekly cross-disciplinary team huddles.
• Coach CHWs with mentorship and occasional field shadowing.
• Manage staffing schedules for patient coverage and continuity.
• Review dashboards to track CHW performance against goals.
• Partner with Ops/LD on hiring, training, deployment; community reps.

🎯 Requirements

• Experience supervising CHWs or related local health workforces.
• Experience delivering CHW health services; able to support a small caseload.
• Knowledge of Medicaid and barriers to care; external representation.
• Strong judgment; empathy in managing teams.
• Tech savvy: virtual communication, scheduling, and documentation.
• Willing to travel in Sonoma County; valid driver's license and insured vehicle.
