📋 Description

• Conduct face-to-face and telephone outreach to Medicaid members around preventive health, care
• Educate members on the importance of preventive healthcare services, assess opportunities for
• Coordinate with members, healthcare providers, and community organizations to help individuals
• Build trusting relationships with members and empower them to take an active role in managing their
• Develop partnerships with community-based organizations and human services providers, including
• Conduct Social Determinants of Health assessments when appropriate and help members address

🎯 Requirements

• At least 1 year of experience in a healthcare-related customer service, outreach, or marketing
• At least 1 year of experience as a Community Health Worker or in a closely related field.
• High School diploma or GED.
• Must obtain Certified Community Health Worker (CCHW) certification through the Pennsylvania
• Demonstrated knowledge of customer service principles, including understanding customer needs and
• Effective communication with members, peers, supervisors, providers, and community partners while
