📋 Description

• Attend a 2-3 week paid training program.
• Meet patients in the community, home, and facilities; assess needs.
• Help patients with social needs like homelessness, addiction, hunger.
• Work with CHWs and staff to build a directory of community resources.
• Navigate technology systems to document each patient encounter.
• Meet patients virtually, by phone or video as appropriate.

🎯 Requirements

• Highly organized and self-motivated to work independently.
• Sound judgment; quickly analyze situations.
• Ability to work with diverse communities empathetically and professionally.
• Must be available to work weekends and one late-start shift weekly.
• Travel within surrounding counties up to 80%.
• Current driver’s license and access to an insured vehicle.
