Community Registered Nurse (RN) Care Manager

📋 Description

• Conduct nursing assessments in homes and communities
• Partner with NPs and Medical Directors for high-risk patients
• Coordinate care with specialists, PCPs, hospitals, and plans
• Educate on chronic disease management and preventive care
• Support transitions of care after hospital visits
• Document patient interactions using available tech

🎯 Requirements

• Active, unrestricted RN license
• Minimum of 3 years RN experience
• Independent work in community/field settings preferred
• Experience in care/case management or value-based care preferred
• Strong communication and relationship-building skills
• Experience with EHRs and healthcare technology
