Community Registered Nurse (RN) Care Manager

📋 Description

• Conduct comprehensive nursing assessments in patients' homes and社区 settings
• Coordinate care with specialists, primary care providers, hospitals
• Support transitions of care after hospitalizations or ED visits
• Educate patients on chronic disease management and preventive care
• Build long-term patient and family relationships
• Document interactions using available health tech platforms

🎯 Requirements

• Active, unrestricted Registered Nurse (RN) license
• Minimum of 3 years RN experience
• Experience in care management or chronic disease management preferred
• Strong communication and organization skills
• Experience with EHRs and healthcare tech
• Valid driver's license and reliable transportation
