Conduct comprehensive nursing assessments in patients' homes and community settings
Partner with NPs and Medical Directors to support high-risk patients
Coordinate care with specialists, primary care providers, hospitals, health plans, and community
Educate patients on chronic disease mgmt, meds adherence, preventive care
Support transitions of care after hospitalizations or ED visits
Identify and close quality care gaps via outreach and coordination

🎯 Requirements

Active, unrestricted RN license
Minimum of 3 years RN experience
Independent work in community/field settings preferred
Care management/primary care experience preferred
Strong communication, organization, relationship skills
Experience with EHRs and healthcare tech
