Complex Care Specialist

📋 Description

• Independently manage a caseload of complex, multi-need clients via long-term case management
• Facilitate engagement as clients move through care levels using Motivational Interviewing
• Manage client crises with risk/needs assessments, safety planning, and de-escalation
• Communicate with clients and providers via email, phone, and occasional text
• Use Lyra's proprietary matching software and other resources to identify treatment options
• Administer and monitor symptoms with PHQ-9, GAD-7, or other measures

🎯 Requirements

• Master’s in clinical social work or equivalent; LCSW or independent licensure required
• Experience managing high-risk clinical situations and health plan navigation
• Proficient in case management and delivering specialized care for diverse needs
• Knowledge of higher levels of care and community mental health resources
• Self-directed, remote-work capable professional and collaborative team player
• Familiarity with Health Plan procedures and medical necessity criteria
