Complex Care Specialist - Bilingual Spanish Speaking

📋 Description

• Assess client clinical needs for higher levels of care
• Provide phone/email support for complex behavioral health
• Ensure client safety; navigate clients to high-quality care
• Deliver longitudinal case management remotely
• Work independently and with a small team of clinicians
• Spanish-speaking; US resident; shift Mon-Fri 9:00-5:30 PT

🎯 Requirements

• MSW or equivalent; LCSW or independent licensure
• Licensure to deliver mental health services independently
• Experience with high-risk clinical situations
• Case management for diverse care needs
• Knowledge of higher levels of care and community resources
• Remote, self-directed professional; collaborates with a team
