Direct Support Professional (DSP) - Community Connections Program

📋 Description

• Deliver direct support per ISP, BSP, and Nursing Support Plan.
• Facilitate structured community activities to promote participation.
• Transport individuals and coordinate community outings.
• Support participation in events and document services.
• Promote health, safety, dignity, and self-determination.
• Promote community integration and skill development.

🎯 Requirements

• 1+ years experience with individuals with IDD, mental health or related human services.
• Valid driver's license and vehicle with auto insurance.
• Must be at least 18 years old.
• Must be able to provide references.
• Able to attend two weeks of PAID in-house orientation and training.
• People-oriented with a team approach.
