📋 Description

• Partners closely with the Outreach Specialist, Enrollment Specialist, and Interdisciplinary Team
• Conducts comprehensive nursing, home safety, and eligibility assessments in participants' homes and
• Completes Nursing Facility Level of Care (NFLOC) assessments and related documentation in
• Participates in enrollment eligibility meetings by presenting clinical findings and collaborating
• Conducts home safety evaluations of the prospective participant’s home.
• Builds trusted relationships with prospective participants and caregivers by providing education

🎯 Requirements

• Graduate of an accredited school of nursing (ADN or BSN)
• Current, unencumbered RN license in the state of California
• Minimum of 1 year of experience working with the frail or elderly in a community-based setting
• Experience conducting nursing assessments and familiarity with Level of Care (LOC) determinations
• Strong clinical assessment, communication, interpersonal, and documentation skills
• Working knowledge of Microsoft Office Suite, electronic health records (EHR), and documentation
