Hybrid clinical role providing care digitally and in person
Monthly travel to onsite employer clinics in Bastrop, TX and McGregor, TX (2-3 consecutive days)
Remote follow-up care and visits with virtual national network
Care for patients aged 0-100 with chronic conditions and preventive care

🎯 Requirements

5+ years of clinical experience in chronic and primary care
Board certification in Family Medicine
Active Texas license or ability to obtain by start date
Experience with digital-first healthcare and virtual medicine
Strong communication; English/Spanish bilingual is a plus
Proficiency with proprietary clinician-patient platform, Google Suite, Zoom, Slack
