Family Medicine Physician

📋 Description

• Act as a key PCP for our patient population, delivering care in the clinic and digitally
• Provide primary care across preventive health, chronic disease management, urgent care, and
• Collaborate with care team members including behavioral health, virtual urgent care and
• Educate patients and develop individualized care plans
• Collaborate with team to solve patient problems using resourcefulness and a can-do approach

🎯 Requirements

• 5+ years of clinical experience in chronic and primary care for all ages
• Active board certification in Family Medicine with comfort seeing Pediatric patients
• Active licensure in Texas or ability to obtain license by start date with Galileo
• Strong communication skills; translate complex ideas into simple language
• Bi-lingual in English and Spanish is a plus
