📋 Description

• Manage an assigned patient's caseload
• Adhere to a revolving monthly patient's service and compliance standard
• Direct point-of-contact for enrolled member questions, inquiries or concerns
• Experience with data input, case review, and building rapport with patients and caregivers
• Adept at multitasking with strong written and verbal communication and organization
• Collaborate with Care Coordinators to schedule home visits using tools for efficiency

🎯 Requirements

• 1+ years of experience in field-based roles
• Capable of assessing patient health risks and identifying potential hazards within the home environment
• Motivation to meet and/or exceed case management targets or metrics
• Excellent written and verbal communication skills
• Excellent time management, critical thinking, and active listening skills; adept at multitasking
• Must have your own reliable transportation and ability to travel every day
