Field Case Manager

📋 Description

• Serve as the primary contact for physicians and healthcare provider teams within the assigned
• Communicate with patients and program stakeholders as required, developing and maintaining trusted
• Educate healthcare providers and their staff about program services, patient support resources
• Review patient charts and ensure all required documentation is accurate and complete, including
• Take ownership of the special authorization process, ensuring submissions are timely, accurate, and
• Provide physicians with ongoing updates and guidance regarding patient status, program changes

🎯 Requirements

• Registered Nurse (RN), Bachelor of Science in Pharmacy, another relevant science degree, or
• Knowledge of Canadian public and/or private third-party reimbursement environments.
• Previous case management experience is an asset.
• Bilingual English and French communication skills, both written and verbal, are required for
• Strong understanding of medical terminology and the ability to communicate confidently with
• Exceptional interpersonal and customer-service skills, with the ability to establish credibility
