• The Field Case Manager is responsible for managing comprehensive patient care plans, coordinating

🎯 Requirements

• Bachelor’s degree in Nursing (BSN) or a related healthcare field required; a Master’s degree is
• Current and unrestricted Registered Nurse (RN) license in the province of employment.
• 3–5 years of clinical experience, preferably in case management, home healthcare, or a related
• Strong understanding of healthcare systems, insurance processes, and available community resources.
• Excellent assessment, critical thinking, care planning, and problem-solving skills.
• Strong interpersonal, communication, and negotiation abilities when working with patients and
