• Conduct initial and ongoing assessments to determine participant needs, strengths, and barriers to
• Assign participants to appropriate W-2 service tracks based on eligibility and assessment outcomes.
• Collaborate with families to establish employment and self-sufficiency goals, prioritizing barrier
• Develop and implement wraparound strategies including job readiness, career planning, budgeting
• Coordinate activities outlined in the W-2 Individual Employment Plan (IEP) with those of external
• Monitor participant progress, update service plans, and adjust strategies as needed to ensure goal

🎯 Requirements

• Associate’s degree in human services, Social Work, or related field.
• Minimum of 2 years of experience in case management, employment services, or social services.
• Strong organizational and communication skills.
• Ability to work collaboratively with diverse populations and service providers.
• Proficiency in Microsoft Office and case management software.
• Bachelor’s degree in a related field (preferred).
