Full Time Registered Nurse-Home Health Visits

📋 Description

• Provide home health RN visits across Berks County per care plans.
• 1:1 patient care with flexible schedules and work/life balance.
• Follow designated care plans and document interventions on a touch pad tablet.
• Coordinate case management and communicate with Clinical/Client Services teams.
• Travel to patient cases as assigned within geographic territories.

🎯 Requirements

• Current PA Registered Nurse license.
• Minimum 1 year of recent clinical (medical/surgical) experience.
• Home care experience preferred but not required.
• Graduate from accredited nursing program; ability to work independently.
• Strong interpersonal and computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
