Geriatric Care Manager (LPN, RN, or Social Worker)

📋 Description

• Develop and manage individualized, comprehensive care plans
• Support patients and families with advance care planning discussions and goal setting
• Execute CoCM model and implement behavioral health interventions
• Collaborate with MDs, CNPs, and PAs to update care strategies
• Identify and close clinical and documentation gaps
• Manage hospital and rehab discharge coordination to prevent 30-day readmissions

🎯 Requirements

• Bachelor's degree or higher preferred
• Licensed personnel preferred - LPN, RN, or Social Worker
• 3–5 years direct clinical experience in geriatric, acute, post-acute care, or discharge planning
• Experience in behavioral health preferred
• Knowledge of Assisted Living and Memory Care communities
• Ability to work independently with time-management and organizational skills
