Health Coach Specialist I (Southfield MI / Field Based)

📋 Description

• Schedule and perform initial home assessments (virtual and in-person)
• Serve as field expert for waiver onboarding and orientation
• Perform required home inspections for safety/compliance
• Partner with Care team to monitor Daily Notes in Salesforce
• Maintain caregiver contact as needed; document in Salesforce
• Conduct semi-annual visits with Waiver Case Manager

🎯 Requirements

• 1+ years experience with frail/elderly adults and caregivers
• 1-3 years medical field/case management experience
• Ability to exercise judgement independently; raise issues
• Strong verbal and written communication; build rapport
• Strong problem-solving; identify issues and escalate
• Excellent time management and active listening; multitask
