📋 Description

• Orient new patients and assess program fit
• Train caregivers to deliver high-quality care
• Perform home orientations and paperwork
• Initial patient assessments and ongoing visits
• Travel within Atlanta metro area for fieldwork
• Collaborate with care managers and staff

🎯 Requirements

• 2+ years with frail or elderly disabled adults and caregivers
• Working knowledge of ADL/IADL assessments
• Experience with data entry; Salesforce a plus
• Proficient in Microsoft Word, Excel and Outlook
• Excellent verbal/written communication; strong organization and time management
• Valid driver's license and insured vehicle for GA travel
